Once your form has been completed, it will be reviewed, approved, and processed.
You will be notified once this is completed.  

#2 Add Coach Jodi to your E-Mail contacts

It is super frustrating for everyone when you are expecting an e-mail and it never comes because it landed in your spam folder.  Let's eliminate this hassle before it even happens.  Go put this email address into your contacts so you don't miss anything important:

#3 After you are approved

AFTER you receive notification from CHIROTHINTRACKER that your Health Intake Form has been approved, it is time to get you all the way ready for this journey.  Put your name and best email in and click SUBMIT!  You're almost there!!!
